Objectives:The aim of this study was to provide a quantitative evaluation of the effects of Lasix on renal function in patients with acute kidney injury.
Methods:A total of 12 patients with acute renal injury (n=12) and 9 healthy male volunteers (n=9) were enrolled in this study. The study was carried out in accordance with the Declaration of Helsinki. The patients were admitted to the hospital with a diagnosis of acute interstitial nephritis, and they were diagnosed with acute kidney injury (AKI). Patients were evaluated for renal function by the Renal Function Assessment Scale (RFS) and their clinical and laboratory parameters were evaluated. Patients were also tested for diuresis using a modified forced response test (FDRT) and their serum electrolytes were analyzed to determine the electrolytes which were the most important in the acute kidney injury. All the patients were followed up at the outpatient clinic of the University of São Paulo (PSP), and the study was approved by the local Ethics Committee. The ethics committee waived this request and written informed consent was obtained from all participants. This study was approved by the Clinical and Laboratory Review Board of the University of São Paulo (UPSP).
Results:Among the 9 patients, 7 (70%) had a history of renal disease; however, 5 of them were found to have no renal function abnormalities. There were no patients with CKD; however, there were 6 patients with AKI. Moreover, in this study, the mean values of the RFS were 28.0 (± 10.0) and 34.5 (± 9.0) in the patients with AKI, respectively. The mean serum electrolyte level was lower in the patients with AKI (5.7 (± 1.6) mmol/l) (p=0.037) and in the patients with CKD (8.1 (± 2.2) mmol/l) (p=0.025).
Conclusion:The effects of Lasix on renal function were observed in patients with acute kidney injury. Moreover, there were no significant differences in the electrolyte levels of the patients with AKI and CKD. Therefore, the use of Lasix in patients with AKI is not recommended.
Patients with acute interstitial nephritisIn this study, patients with AKI were evaluated for renal function by the RFS and the FDRT. The patients with AKI were divided into two groups based on their serum electrolytes. In Group 1, the patients with AKI had a mean value of 5.7 (± 1.6) mmol/l, which was significantly lower than the values in Group 2 (2.8 (± 1.8) mmol/l). In Group 2, the patients with AKI had a mean value of 4.1 (± 1.3) mmol/l, which was significantly lower than the values in Group 1 (2.6 (± 1.3) mmol/l). There was no significant difference between the two groups. No significant differences were observed between Group 1 and Group 2 in the serum electrolytes of patients with AKI. The patients in both groups with AKI had a mean value of 5.0 (± 1.5) mmol/l, which was significantly lower than the values in Group 1 (2.2 (± 1.5) mmol/l) (p=0.005). The patients in both groups with AKI had a mean value of 4.3 (± 1.8) mmol/l, which was significantly lower than the values in Group 1 (2.3 (± 1.4) mmol/l) (p=0.002).
